The LLPS is a provincial initiative aimed at reducing avoidable, non-traumatic limb amputations caused by vascular disease and diabetes. The Windsor Team Care Centre is participating in this initiative and has established the Lower Limb Wound Prevention & Treatment Clinic offering comprehensive care, including wound management, personalized treatment plans, and educational resources to support lower limb health.

The Lower Limb Wound Prevention & Treatment Clinic is designed for individuals at high risk of limb amputations, particularly those dealing with complications from diabetes and vascular disease. This includes:

• Individuals with diabetes who are at risk of foot ulcers and other lower limb complications
• Patients suffering from peripheral artery disease (PAD) and other vascular conditions affecting the lower limbs
• Homeless and street-involved individuals with limited access to healthcare
• Indigenous communities experiencing higher rates of lower limb complications
• Residents of high-priority neighborhoods with elevated amputation rates
• Patients of the Windsor Family Health Team (WFHT) and the Windsor Team Care Centre (TCC)

When you visit the Lower Limb Wound Prevention & Treatment Clinic, you can expect:

1. Comprehensive Assessment: Our team, led by experienced Nurse Practitioners, will conduct thorough assessments of your lower limb health, including wound evaluations and risk assessments for limb loss.
2. Personalized Treatment Plans: Based on your assessment, we will develop a personalized care plan that addresses your specific needs, including wound care, dressing changes, comorbidity management, and referrals to additional services if necessary.
3. Integrated Care Coordination: We collaborate with various health and social service providers to ensure you receive holistic care. This includes coordinated follow-ups and facilitating access to preventive and investigative services.
4. Support and Supplies: We provide essential supplies such as diabetic socks, compression stockings, and appropriate footwear to support your lower limb health. Our team also offers immediate care for urgent primary care needs that may arise during your visit.
5. Educational Resources: To empower you in managing your health, we offer educational sessions and self-service modules that cover various aspects of lower limb care, including prevention strategies and self-care techniques.

Primary Care Providers can refer patients to the Lower Limb Wound Prevention & Treatment Clinic using our dedicated referral form. Our aim is to see and assess referred clients within 72 hours of receipt of a referral.

For clients without a primary care provider or are who are experiencing homelessness, can attend our walk in clinic at the Housing and Homelessness Help Hub (H4).
